The Transitional Support Program provides property and tenancy management while clients engage with case management support. This program supports families, single adults and young people experiencing homelessness. Support workers provide holistic case management to address housing goals, mental and physical health and wellbeing needs, parenting and family support. The program focuses on capacity-building to help avoid future episodes of homelessness.
Clients are offered medium-term support to access and maintain longer-term housing. The focus is on securing or maintaining safe and stable accommodation, and on strengthening links with community supports.
It also supports families staying in transitional housing and provides assertive outreach, to help link them in to supports while they are waiting for a more secure and appropriate housing option.
Launch Housing manages 560 transitional housing properties.
