Housing & Mental Health

Launch Housing’s mental health housing services support people with mental ill-health to find and secure permanent homes.

Our support workers partner with real estate agents to assist at risk single, couples and families that are sleeping rough, living in crisis accommodation, or transitional housing.

Clients are supported to find and establish a tenancy, with the aim to empower individuals to maintain their own lease, develop local networks, and live as independently as possible.

Access Our Mental Health Housing Services

To access this service you must first go to a Launch Housing office (access point) at Collingwood, St Kilda and Cheltenham. At an access point you will be given an assessment, housing information and accommodation assistance.

Opening Hours: Monday to Friday, 9am to 5pm.

Housing Mental Health Pathways Program

The Housing Mental Health Pathways Program (HMHPP) supports acute mental health patients with a history of homelessness who have no suitable accommodation at the time of discharge from hospital. We aim to support and care people by offering emergency shelter, crisis accommodation and housing. This program supports patients of St Vincent’s and The Alfred Hospital’s psychiatric wards.

Referrals

Referrals are made by social workers at the respective inpatient units.
